California's largest immigration detention center has significantly failed to provide adequate healthcare, falling short of a judge's orders and placing detainees at serious risk, a court-appointed monitor found.

The California City detention facility, situated northeast of Los Angeles, was found to have critical flaws across its healthcare system. These deficiencies include major gaps in emergency care, delays in providing essential medications, inadequate initial health screenings, and a lack of necessary accommodations for individuals with disabilities. The monitor's report, filed with the court on Monday, stated the facility "lacks a reliable system to consistently provide adequate health care, placing individuals at serious risk of both immediate and long-term harm."

The investigation was mandated by a federal judge in February following a lawsuit alleging widespread medical neglect. The judge required the government to prove it was meeting required medical treatment standards in eight key components of the facility's healthcare system. The monitor, Dr. Muthusamy Anandkumar, a physician who previously served as a medical expert for the Department of Homeland Security, determined that the California City facility was out of compliance with all eight areas.

These findings mark the first court-ordered investigation into the comprehensive scope of medical care at any Immigration and Customs Enforcement (ICE) facility in the United States. The monitor's conclusions align with significant concerns previously raised by detainees regarding conditions and healthcare over the past year.
