Brain Hormone Imbalance Can Hinder Pregnancy Despite Regular Periods

A woman's menstrual cycle, often assumed to be governed by the uterus and ovaries, can be disrupted by hormonal imbalances originating in the brain, making pregnancy difficult even with regular periods. This occurs when the balance of hormones regulating ovulation is disturbed, preventing ovulation despite the appearance of a menstrual cycle.

One common cause is a prolactin-secreting tumor. A woman in her 30s, after two years of trying to conceive without success, sought medical help for milk production despite not being pregnant or having given birth. Diagnosed with a prolactin-secreting tumor through blood and MRI scans, she successfully conceived naturally after hormone levels normalized with medication.

While many associate infertility with uterine or ovarian issues, hormonal imbalances are a significant factor. Excess prolactin can interfere with ovulation and the implantation of a fertilized egg. Symptoms of elevated prolactin include irregular or absent periods, and unexpected lactation. Men can also experience decreased libido, erectile dysfunction, fatigue, and headaches or vision problems if a tumor grows large.

Diagnosing elevated prolactin is straightforward with a blood test. However, high levels can also result from severe stress, lack of sleep, intense exercise, certain medications, or underlying conditions like hypothyroidism or polycystic ovary syndrome. Doctors first rule out these factors before considering further investigation with an MRI for pituitary tumors.

Most prolactin-secreting tumors are benign and respond well to medication, which lowers prolactin levels and shrinks the tumor. This treatment often leads to restored fertility and successful pregnancies. Experts emphasize that while the term "brain tumor" can be frightening, these tumors are typically treatable and manageable with proper diagnosis and medical care.
